HPC learned that DSI had a permit set dated 11-9-15 when the HPC set of plans were dated 9-29-15 for the revisions to the original permit set submitted to HPC which was 8-19-15. No Certificate of Approval was attached to the permit or plans nor were the plans stamped approved by the HPC. Staff has revised the COA in folder 15-153292 and will forward to the contractor to attached to permit plans. AHS, 2-23-16.
