Employee on Atv Hits a Tree and Dies at the Scene
At approximately 7:50 p.m., on September 20, 2011 Employee #1 was working on a c rew responsible for the operation of the main pump of a dairy farm. Employee #1 was on an all terrain vehicle (ATV) and had just delivered a gasket to repair a leak in one of the above-ground hose couplings out in the field. As he was retur ning to the main pump area, he went off the dirt road into the ditch where the A TV's front right tire struck a tree and ejected him. Employee #1 was thrown from the vehicle, struck a tree, and died as a result of his injuries. Employee #1 h ad not received training on the safe operation of the ATV.
