Employee Falls Off Dock, Fractures Leg and Later Dies
At approximately 3:30 p.m. on May 23, 2011, Employee #1 was picking up a package at a courier warehouse. A warehouse employee saw Employee #1 entered a hallway. No other company employees were on the premises. The two individuals met in the area and the warehouse employee switched on the light to the warehouse/dock are a. They both went into the warehouse, but Employee #1 was unable to find the pac kage he was looking for. The warehouse employee turned off the lights to the are a when he left the building to make a phone call in his vehicle. Employee #1 ree ntered the area without turning on the light switch adjacent to the door to the warehouse/dock area. The employee heard him calling for help within a few minute s. The area was very dark with all doors closed. Employee #1 stated that he had fallen from the edge of the dock in the dark warehouse and had crawled up the st airs adjacent to the dock and yelled for help. The employee heard his calls and provided assistance, and then called emergency medical personnel. When the emplo yee was assisting him, it appeared that Employee #1 had a compound leg fracture. Employee #1 was transported to a medical center, where he received treatment, b ut had succumbed to his injuries and died.
