Employees suffer burns and lacerations due to explosion
On June 10, 2010, Employees #1, #2 and #3 were on top of the Phthalic Anhydride tank when an explosion occurred. One of the employees tack welded a metal cover to a material returning line on a tank that caused a flash explosion. The tank c onsisted of approximately Phthalic Anhydride in its molten state. The tank was 1 9.5 feet high, about 14 feet in diameter and had a capacity of 20,000 gallons. A ll three employees were injured and received medical attention. As the tank expl oded, two employees were sent over guardrails and plunged to lower levels, while the other employee was able to remain on the tank during the explosion. Injurie s included second degree burns, fractures and lacerations.
