Employee Falls and Strikes Head, Lated Died
On February 22, 2007, between 11:30 a.m. and 12:53 p.m. Employee #1 was opening the back doors of the tractor trailer he was delivering. One door was opened and secured and while opening the second door, a bundle of cardboard, weighing appr oximately 400 pounds, fell out, pushing the door and knocking Employee #1 over. Employee #1 then fell back and struck his head on the ground. Employee #1 was ta ken to the hospital when he was discovered. Employee #1 was hospitalized with he ad trauma until he died on March 22, 2007.
