Employee Is Killed in Fall from Elevated Work Platform
At approximately 10:00 a.m. on February 12, 2005, Employee #1 was installing ext erior dense glass sheathing on the third floor of an apartment under constructio n. He was working from an unguarded two foot extension at the front of a work pl atform that was attached to a rough terrain forklift when six sheets of the glas s sheathing tipped over from the back of the work platform. The sheets knocked h im off the platform and he fell approximately 29 feet and was killed. He was not wearing a personal fall arrest system at the time of the accident.
