Employee cruched and killed when caught in machine

Employee #1 entered into an unguarded area on the third floor. She was hit from behind and pinned by the threading bar that was moving film from the first floor up to the third floor and then through the oven. The employee was crushed and k illed. The machine had multiple sets of controls that could activate the machine from different floors. The operators at the various control stations could not see each other. An operator on a lower floor activated the machine while the vic tim was in the hazard area.
