Employee killed when scissors lift falls
At approximately 12:40 p.m. on July 1, 2003, Employee #1 was working under a sus pended scissors lift table. The table had been hoisted to the fully extended pos ition by an overhead chain hoist. The base of the table was resting on the floor . The chain slings were attached to tabs on each side of the table top. The weld s on the tabs broke, the table collapsed, and Employee #1 was crushed and killed . The tabs were .25-in by 3-in by 5-in nominal. The tandem lift table was equipp ed with maintenance bars that were designed to prevent the table from collapsing ; however, the bars were not in place.
