Employee Killed When Concrete Wall Collapsed
On May 28, 2002, Employee #1 was working within 6 ft of the base of a concrete w all preparing the area for the pour of a pad in front of the wall. The wall had been poured on May 24, 2002, and the formwork was removed on May 24, 2002. Backf illing of the wall took place on May 27, 2002, with dredged sand. At approximate ly 7:30 a.m. on May 28, 2002, a garden hose was used to soak and consolidate the backfill. At approximately 8:18 a.m. the poured in place concrete wall collapse d onto Employee #1 crushing him. At the time of the wall collapse no bracing was in place. Employee #1 was killed.
