Employee killed when struck by falling load of lumber
At approximately 2:10 p.m. on January 22, 1998, Employee #1, a carpenter, was wo rking in the vicinity of a Pettibone forklift with a platform on the elevated fo rks. The platform was carrying two coworkers, thirty 4 by 8 sheets of 7/16 in. O xboard stacked on the floor, and approximately ten 14 in. long 2 by 6 boards res ting on top of the guardrails. Another coworker was lowering the platform when t he forks tilted forward, causing about 21 sheets of the Oxboard and half of the 2 by 6 boards to slide forward and out of the platform. The unsecured platform a lso slid forward, but did not come off the forks. As the sheets of Oxboard moved forward, they hit a wall and then fell, striking Employee #1 on the side of his head. Emergency services was called and paramedics arrived on the scene within minutes. Employee #1 was transported to the regional hospital, where he was pron ounced dead on arrival due to closed head trauma.
