Employee killed in fall from fixed ladder

Employee #1 was making a routine check of a lift station (underground sewer stat ion). The employee was descending a 28 ft fixed ladder when he fell and struck h is head against concrete at the ladder base. Employee #1 suffered massive head i njuries and died from the loss of blood. The lift station and ladder was manufac tured by Smith Loveless, located in Lenexa, KS.
